Purpose: Use this form to request records from the Department of Motor Vehicles (DMV) in
accordance with the Virginia Freedom of Information Act
(FOIA) or for civil immigration enforcement purposes.
Instructions: Provide as much information as possible. DMV must be able to identify and
locate the records you request. If you are requesting driver, vehicle, or personal records, including crash
reports, submit a completed Information Request form (CRD93)
on the next page.
Limitation: Your request must ask for existing records or documents. FOIA gives you the
right to inspect or copy records but does not require DMV to create a record that does not exist. If you

Fees:
DMV is allowed to make reasonable charges not to exceed its actual cost incurred in accessing, duplicating,
supplying, or searching for requested records. You may request a cost estimate in advance. If you do not
request a cost estimate, DMV will proceed to process your request and notify you of any fees with our
response.
